Brazosport ISD has teamed up with Baylor St. Luke's Medical Group, Baylor College of Medicine, and St. Luke's Health Brazosport Hospital to offer physicals for all students participating in UIL events. Students that are entering 7th grade, incoming 9th grade, high school juniors, and new students to the district are required to have a physical on file.
All BISD Student who need a physical are welcome.
Parents please join your child through the process! If a parent is unable to accompany their child, the permission slip and Medical History Form MUST be filled out and signed at the bottom PRIOR to arrival. Otherwise, the child will be turned away.
Physical forms are available from Athletic Trainers at both high schools or can be printed off by going to
